Depending on how your results were submitted can determine why your results have not yet posted.
Health Provider Screening Form
If you faxed a health provider screening form there are multiple reasons why a form may not process, which includes but is not limited to
- A fax cover page was included
- It is not legible
- The specific printing instructions were not followed or
- Wrong fax number.
If you faxed your form and your results have still not been posted to your account, please go ahead and submit your form using the submission link found on the biometric screening tile. After uploading the form you can expect your results to be posted within 72 business hours. Please be sure all information is accurate and legible or there is risk your form will not be able to be processed.
If you attended an onsite screening event and your results have not been posted to your account, please send us the number found in the upper right corner of the score card you received at the onsite event. We will then track your results and have them posted to your account.
If you visited a participating Quest or LabCorp facility and your results have not been posted to your account, we first ask that you reach out to Quest or LabCorp and ensure they have recorded your results.
If confirmed that your results have been recorded, please tell us the date of your screening, the name and location of the facility, and the full name used for the screening. We will then work with Quest or LabCorp to ensure they have sent us your results.
